Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. If you have dental insurance that can be confirmed, we are happy to help you with this and submit it for you. We will normally only ask that you pay the portion that we expect insurance not to cover. If you have questions regarding this or need to have an estimate prior to having surgery, we will ask that you come in for a consultation appointment to determine what you need to have done and what we believe that your out of pocket expense will be. We can only give you an estimate and will not guarantee insurance payments or amounts. We accept all forms of payment - cash, check or most credit cards.
Dental Insurance
We recommend that you contact your insurance company prior to your office visit. There are as many different insurance plans as there are employers, therefore it is your responsibility to verify the specifics of your plan's coverage. Questions to ask your insurance company include your plan yearly maximum, the percentage of coverage and how much of your yearly maximum you have already used. Most times what you have done in our office falls under your dental insurance plan. On rare occasions it could be covered under a medical plan. The things normally covered under a medical plan are cancer surgeries or trauma due to an accident. Wisdom teeth extractions and other dental surgical procedures are not normally covered under the medical insurance plan. We ask that you check with your plan if you have questions regarding this. We will be happy to help you as much as we can with your insurance questions, however time limitations and plan specifics can sometimes interfere.
As of April 1, 2023 we will no longer be in network with Aetna Medical.
